The Activate option need to appear if the user presses the button after deactivation is completed

Hi everyone, I have a requirement for Temporary Call Divert using Declarative Actions and UI Builder page. I have two fields: a Diversion number that the user enters, and a Type of Action choice field that can contain Activate, Modify, and Deactivate. An RITM and Request would be made for each type of action.

 

Now, just the Activate option need to appear if the user presses the button after deactivation is completed. How to use UI Builder to accomplish this.

 

Note: There may be more than one modify request.
