activity actions playbook UI builder

I have created "Submit " activity action button on the playbook activity 

and in the server script section of the activity action i have added gs.addInfoMessage("test")

currently it is showing two times in the workspace once i clicked on the submit button i wanted to show it only once ??

is the form getting submitted more than once?
