Activity feed not getting updated with notes!!

Hi Team,

 

We are trying to create equipment instance using change request, instance is getting created.

 

But Activity feed work notes not getting updated for 1st instance when its created.

 

For 2nd onwards notes coming correctly 

 

What would be the issue? Please  help
