If a user selects "Create New O365 Shared Mailbox" under "What would you like to do" On the form below an approval needs to be sent to an approval group. The second screen shot is the workflow. I do not know how to add a task to send the approval. I already set up the approval group. Thanks.

Use if condition from the core activities and then use Approval - Group,
Condition - "What would you like to do" is "Create New O365 Shared Mailbox"
