You might have a BR with .update() in it, changing the BR to run as async might work for you depending on the use case for your BR. Here is more info from ServiceNow Support: https://support.servicenow.com/kb?id=kb_article_view&sysparm_article=KB0870192

Hi @MustaphaB
Without looking into your instance its tough to suggest what would be probable cause for this,
As @jonsan09 suggested have a look at your business rules as that can be one of main reason.
Also Review Business Rules, Flows, and any catalog logic touching the comments field to identify the duplicate .
