Adhoc approval for ER cases

I tried adding an ad-hoc approval to an Employee Relations (ER) case. It shows as successfully added, but nothing appears in the Approver related list, and no record is created in the Approval table.

However, the same functionality works correctly for standard HR cases.

Can anyone please tell me how to fix this? Why it's not working for ER cases? I tried using script tracer but nothing found.

If approval is not generated in sysapproval_approver table, can you tell the steps on how you generated approval? Is it via script or etc if possible with screenshots
