ASYNC Business rule is stuck and running for last 3 days

Dear Experts,

 

There is a BR in "sysapproval_approver" table which update worknotes on ritm based on approval state. But it is running for last 3 days and has not updated the comments on RITM also. We faced this issue in past also but that time we killed the transaction. This time we want to find the root cause.

Would you please help me to find the possible reasons for that.
