Bulding new Change Form for another team in organization

Currently we are using Change Managment for all IT related changes I have another team in our organization that is wanting to use the Change process for their changes. 

 Is this possible and if so do I need to create a new change model?  I have create this below:

The issue I am having is that I need to customize the Change Form to the fields that my other team needs and not use the same fields our IT department is using. 

 

Any help would be greatly appreciated I am STUCK!

I worked as Change manager and i can share my thoughts onnthis. 1st thing is, make sure across your org same change form is used, hiding few field for team will increase technical debt for you. And once you done with this then change model is best option.

Thanks for your advise. It would be great to have all teams use the same form but its not possible.

If the approval process is the same as the normal change process your IT changes go through and the fields you don't wish to use for the other teams in the organisation aren't mandatory you could just create a new view for those teams to use that only show the mandatory fields as well as any others they want, then just don't add the ones they don't want.

You can also provide those users a link that opened a new change in the view that they use, so they don't need to manually select the correct view.

 

Of course if they don't want to use fields that are mandatory that's a whole different issue.
