Can we add KB article on report and issue on portal?

Just like Incident can we have a option to populate kb article based on short description on portal?

Yes

it is OOTB, use the contextual search.

It is OOTB

 

Go to Record producer in Navigator

Search for create Incident

In variable 

click on Contextual Search
