Can we get the description values of change request values in the std_change_proposal ?

From the std_change_proposal form, I need to get the values of Change request values and populate that in normal fields in the same table.. As this is the first stage of new template creation, I am struggling to get the values.

If that is not possible, please guide the way to get the value of change request values of "Propose a new standard change template" record producer , therefore I can use that values to map to std_change_proposal form.
