catalog Builder : Lookup with Record reference based on a specific field of the table (eg Full Name

We have a location table with :
Store Nr
Full Name
Street

City

....

I want to create a form where they can lookup based on Full Name 
And show with Auto-populate the street and the city.

Unfortunatly this work if I look up based on Node nr, but not if I want to look up based on Full Name.

I can do this with a dropdown based on value, but then I do not have the auto populate anymore.

Anyone an idea?

I understand that it is a possible solution, but I do not want to script with catalog builder....
