Copy attachment from interaction chat to Request

whenever agent creates any request, all the attachment should be copied from interaction chat to request (REQ/RITM).

How to do this via flow designer.

what can be the trigger condition and how to look up record

In triggger i gave interaction_related_record table only.  1.after trigger my next condition is 'look up records' there i am giving sys_attachment table and adding this script ( as my requirement is duplicate should also be prevented from adding.)
2. after this I am giving IF condition where look up attachment records - is not empty
3. for each condition for attachment records
4. copy attachment  

but getting this error

I could see Target Record value is empty, please check that!
