Create a custom GlideModal popup to apply templates

Hello - We have a requirement to turn off the "Toggle Template Bar" in the Incident form and create a new UI action which will open a new GlideModal popup to show the list of existing template and by selecting it, it should apply the template to the incident form. How can I achieve that? Please assist

This post will tell you how to remove the "Toggle Template Bar" from the options. With that said... I would push back on this requirement. It's going to incur a lot of technical debt for practically no gain.
