Create an incident when task is closed incomplete

Hi,

What is the best way to create an incident when catalog task fails?

Does it have to be configured in the flow?

Example: A catalog tasks has 5 sequential tasks, if any of them fails (is set to closed incomplete) there will be an incident created.
