Create user through Service Catalog in Flow Designer in Microsoft AD Spoke

Hi,

Task is to create a user in Microsoft AD spoke by entering details through Service Catalog (form level) using Flow Designer powershell script. After creating user details in AD, user details should be updated in RITM.

I have created a Flow Designer > Activated it > Through catalog item in portal entered details for new user > REQ was generated and RITM > but user is not visible in sys_user table and error in flow is there and screenshot is attached below.

What could be the issue as I am not able to proceed
