Created Date Displayed in Form Header

I created a new table so I can reference the values in several other places.   For some reason the form header displays the form name along with a date. I cannot find how to remove the date or why it is showing:

Probably because you don't have any column set as the display value, can you confirm if this is true or not?   Tested in demo10 and if I make a new table with no custom fields, just system generated fields, it seems to be defaulting to the created date, but when I made a new custom field and made it the display value, the form header started showing the custom field information.

Hi Chris,

I thought that was the problem and indicated a display value, but the date was still showing. I just checked and it must have taken a while to update because it looks good now.
