Filing in a field from a form

Hi there, 

 

I have a requirement where I have a 'cancel reason' field on a form. I would like a popup when the user clicks on the 'cancel' button, and for a pop-up to appear asking for a cancellation reason. Once the user types in the reason I want it to populate the 'cancel reason' field on the form. 

 

How would I go about doing this in a simple way?

I think I already replied on this,

 

Instead of creating a Pop up , and adding UI page, better

make Cancel reason mandatory when a user select the cancel button. It is easy and straight forward,
