Flow Designer giving error on Run As: System User

We have a flow that is throwing an error in Create record for sys_user_grmember table.

Error:

Although we are encountering this error, the group was still added in users' profile so this does not make sense.

In our properties, the Run As is set to System User as well. 

How can we eliminate the error that is happening on this flow?

Please check the System Log for more details about the error.

Most likely the user is already a member of the group that the Flow is trying to add them to.

Check ACLs, may require to run as 'admin'.

Is it the ACLs for sys_user_grmember?

If you are trying to add same user again to that group then it will throw the error

please check that and share the updates
