Flow "Ask for approval" Manual user getting skipped

I am trying to create a flow for Normal changes, where in Access stage first it looks for Change approval policies for approval. If there is no approval as per change approval policy, then it skips and look for manual approval. Approver is set manually after change is moved to access state. Here in the flow I try to use "Ask for approval" activity to get manual approver. But while creating a ticket and moved it to access state, the flow skips the manual approver stage in flow and gets completed. It doesn't wait for manual approver to be added. What could be the reason?

Hello Imran, 

A manual approver is a user manually added to the Approvers related list who can then approve the request. For example, you can manually add a subject matter expert to a task to approve the request. Thanks for your response. Yes, Manual approver I am using it to add manual approver over here.

If you see the screenshot, first we look through the change approval policy. If the approval policy is not met and when it is skipped, we turn to manual approval. When we try to run this by creating a change, the flow bypasses the "Ask for approval" manual approval without waiting for the Manual approver being added. And it shows with completed state. If you look through output data, it shows state is skipped.
