Generating SLA Report as PDF

Need to generate SLA report. That should be in PDF in below attached format. 

It will be great if i get the solution for this problem, and thanks in advance.

You can create a report on task_sla table as below (you wouldn't have exact fields in out of the box report)

You can export as PDF

The attached report example that you have given is not possible in ServiceNow. You can instead either create reports through the report designer or through performance analytics to get the metrics and visualization that you need.
