Group roles missing from group member

I had a user report a problem accessing some table data they normally have access to and is even displayed on their default dashboard. It turned out that although they were part of our default ITIL role group, the inherited roles were suddenly missing. Resolution was even as simple as removing them from the group and re-adding them.
This is not the first time I've encountered this, but I've not seen this for several months or a year so it is not frequent. In fact the last time I saw this issue it was while working for another company. However, this bug/issue has been around for years, maybe 6+ even as I've seen it multiple times in my career.
Has anyone found a known issue related to this or knows a way to track down clues as to how it happened?

Do either of you have the 'Contextual Security: Role Management V2' plugin activated? It has a fix script to "clean up" inheritance problems. Activating that plugin helps. Create a Support case and they can run that in 'dry-run' mode to point out inconsistancies.
