Guidance Required on Using Assessment Metric Templates for Survey Questions

I am trying to create a survey question in ServiceNow using Assessment Metric Templates. For this requirement, I created two assessment metric templates. The first template was created without any display values and is being used as the parent or main template. Using this template, I created the main question for Adoption & Usage Maturity (Module-wise).

The second template was created with display values such as Fully Adopted, Largely Adopted, Partially Adopted, Low Adoption, and Not Implemented. Using this second template, I created the module-level questions such as Incident Management, Request Management, Change Management, Knowledge Management, Virtual Agent, etc.

In the preview, the survey is rendering correctly and showing the parent and child questions as expected. However, when I open the survey using the survey URL, the layout is different and not rendering in the same way as the preview
I am attaching the screenshots below showing how the question is expected to look and how it is currently appearing in ServiceNow. Please help me understand what might be causing this difference and how I can achieve the expected layout.
