How to activate UI Policy o portal?

Hi,
I have applied an UI policy on Incident form(Self Service View) to make some fields hidden based on one condition.
The UI policy is working fine in the Instance but it is not getting reflected on the Portal.
Portal Image :
In this case of Self service view of form, The fileds 'Category', 'Subcategory', 'Impact', 'Location' and 'Business Service' are hidden in the instance because of the UI Policy but the same is not getting reflected in the portal when I keep the view as 'view=ess' in the portal URL.
Can anyone please suggest me how to apply the UI policy to the Form in the Portal and hide the mentioned Fields.

Normally once the Service Portal plugin is enabled the Service Portal Option should come OOB. Can you please confirm if the Plugin is enabled or is it a separate Custom CMS Portal you are designing?

The Portal is already enabled and i have created the Portal using the Service Portal Plugin.
