How to add Infor Message on the now agent app form

Hi Every one, 

I want to add Info message on the now agent app form, as in the below screen shot.
after clicking the  plus button the start work link button form can open, after clicking the start work link, the Create and start assignment form can open.

in the third form i want an info massage, how to do it.

If I understood correctly, you can do this via client script.

and choose UI type correctly
