How to add link in banner frame and on header bar?

I want to add link in banner frame and a link on header bar like the below images.

A link in banner frame :

Refresh button which will act as a shortcut of 'Reload Form' on header bar :

Please provide me any pointer how to add these two links in any demo instance. Thanks in advance.
