How to add menu items like in a navbar to Service Portal Header?

I want to add menu items like shown below to my portal's header:

These are just examples, I want to add menu items like 'My incidents','My approvals' etc.

How to achieve this ?

What if we do not want to use OOB Header Menu, can anything be done for this ?

Please guide.

If you update the widget and try, does the menu's show up ?, if yes, then you have to look into your custom widget.
