How to add the Submit button to a catalog item

I have a Submit button   on catalog items Hardware Request, Permission Request, Application request form. But it is not on the Report request form from the portal view.   How do I add the Submit to this form from portal view?

Is this "report request form" is a catalog item?   If so, which widget you are using to display this catalog item form?

The reason I am asking is by default if you are using the same widget what you are using for Hardware Request& Permission Request you should see submit button,

post the screenshot of your widget code of report request form if possible.

Can you check if you chose Availability as 'Desktop and Mobile' in your catalog item "report request form". check below screenshot
