Hi Neha,
1. Navigate to - UI Actions (sys_ui_action) table
2. Suppose, Table = Incident
3. Form button = true (You want to show that button on the header of the form)

You can create a new button on incident by creating a new UI Action. See UI Action documentation at the following link.

Hi Neha,
To create a button on form follow the following steps-
1)Open incident form->Go to context Menu ->Configure->UI actions
2)Go to new->create name for it->and mark for Form Button
It will display a button on form .
