How to create a Radio Button on Incident Form

We have a requirement where i need to create a Radio button where user can select anyone from the following:

Hi,

I suggest you use a drop down with the 3 values as you mentioned instead. OOB radio button is not available and if you do some custom macro for this, its not advised and will be a pain in the long run.
