how to create form dialog pop up window in workspace?

Hi Community,

 

I have a requirement to create button on workspace form & when user clicks on that button. a form dialog window should open which consist of one reference field.

Could anyone please help on this?

 

Note;- I tried with UI Action & UI Page but unable to pass current record sys id from UI Action to UI Page. There is something called as Declarative Action in UI Builder which might be helpful but I'm not sure how to implement it.

 

Also tried with:- https://www.youtube.com/watch?v=E9FhNpk0Mwk which says about list dialog window. But not able to configure the same for form dialog window.

 

Kindly help me on this. Any response will be appreciated.
