How to create multiple checkboxes in variable ?

I have requirement where i need to create a variable called ''revision'' as type 'checkbox'. under that i need show 4 options as checkbox, where user is able to select multiple options.

Hi,

You need to create 5 Variables for this. The first is a label "revision"; the other four are four Options. like this

After this create a Catalog UI Policy and apply a filter 

After this right click on Header and click on Save. Now you need to add action, scroll down to the category UI Policy Action Window, click on new, and make one option mandatory.
