How to display additional record fields on assessment in the service portal?

To make this process easier for users, we want to display the current values of these fields directly within the assessment. This way, users will have immediate visibility of each field's existing values and can confirm or update them as needed.
We’d like to display the following information in the designated read area (see image) directly beneath the record name:
Department: [current record's department]
Managed By: [current record's managed_by]
Owned By: [current record's owned_by]
Managed By Group: [current record's managed_by_group]
This setup would provide users with a quick reference to the current values for each of these fields, helping them verify and confirm details efficiently without needing to navigate elsewhere.
