How to fix Catalog form page alignment off

I added the Catalog form into a module within my app and there is an alignment issue within the webpage. Any way to clear this up? I have uploaded what the catalog looks like, vs what the form looks like within the module/menu. Any suggestions to fix this would be greatly appreciated.

Look like some cache or browser issue. Try cache.do and try.

both the images are different, 1 is for portal and 1 is for native

It should ideally be the same look and feel in both

Please adjust your description field
