How to track remediation action metrics ?

Requirement - want to present data to management on metrics on how many times such issues were addressed on the instance performance perspective. To provide data on how many times performance issues (wrt MID and long running transactions) and what was done to maintain instance performance and minimize issues on performance of instance

 

prod instance has MID server issues sometimes a month, sometimes there are long running transactions that need to be checked at killed.

 

How do I track metrics for # of times, MID server was validated and brought back online or long running transactions were addressed to improve instance performance.

 

Maintaining a list with manual effort is not ideal solution. Is there a way to capture on

# how many times MID faced issues and how many times it was solved

# how many time long running transactions were killed
