When I click "Edit Metrics Weight" on the Service Offering form, the modal opens without displaying any data. I checked the UI Action and found that GlideDialogWindow references a specific UI Page, but I couldn’t locate that UI Page in my instance. I need help identifying the referenced UI Page and investigating why the modal isn't displaying the data.

Your screenshots narrow it down. The script calls new GlideModal("modal-weights"), but your UI Pages search on that name comes back empty, so the page it wants to load isn't in the instance. The "Test" title comes from g_form.getValue("name") on the form, not from the page, which is why you get a heading with nothing under it.
So the UI Action is fine. The page it points at is missing.
Check your Upgrade History for anything skipped around that record, and compare with another instance where the button works.

@SiriA9964832329 Could you share the UI Action script, along with a screenshot of the record and the empty modal? Without the UI Page name from the GlideDialogWindow call it’s hard to say whether the page is missing or just failing to load.
