I have a table called Prash. When I click on Show form the below form will open.
Similarly I have created a portal called PrashanthPortal.
And portal page user_profile as an homepage to PrashanthPortal.
All I want is I have to use the above form in user_profile portal page. How to do this.?
Is form widget help me out of this.?

You would need a list widget to utilize the form widget in service portal.
You can add a list widget and add the form under link in instance option of the same.
And if you want it directly, you can hit this URL :
replace your table name and sys_id of the record in the above link.
