How vendor tier and risk rating are determined?

Hello Everyone,

I am trying to explore VRM on Servicenow. the out of the box application comes along with default vendor tier and risk rating scale. Each value on the scale has a range, fir example - if the tier assessment score falls between 60 to 79, then the tier would be High. Similar approach for deriving vendor risk as well.

I defined my own tiering assessment template with two categories and two questions (yes/no) type under each category.

first category's weight is 20 and the next one's is 10

Weight of each question is 10. 
i launched the tier assessment and answered three questions as yes and one as no.
the score came out to be 67.

my questions are below
 
1. how did the score 67 come out? 
2. if we have to define our own scoring scale and not use the out of the box one, then how would be the approach? basically i am trying to figure out the business rules that derive the tier score?
3. in case of risk assessment, when the assessment is completed or closed, the issues are not generated automatically. but in a real world if the vendor responded to the risk assessment questions with answers that are not correct then issues should be auto generated. This feature is not provided out of the box? or are we supposed to script for the same?
 
I would like to have some guidance on these points.
