i wnt to populate task record detais on the survey form on coustome fields

i have created a survey which triggers when an inciden or ritm gets closed now my requirement is i have created to coustom string field on the fields where i want to populate the number and short description on the form but it i am not able to doit how can i acheive it my form is as fallows
