In Utah, the form UI in workspace in Dev instance is different from Test instance

In Utah, the form UI in workspace in Dev instance is different from Test instance. However rest of the things are fine. Could someone point me towards the fix?

Dev workspace form UI :-

Test worksapce form UI :-

 The boxes are not present for input fields on the form. Could someone assist in the soluition.

Hi,

Please check which UI your have on both instances.

There might be a case that on one instance you have Polaris UI (Next Experience) and on another instance you are using UI 16.

Please share which form it is (Table record or Catalog form )

It is a table record. Have checked for the Polaris UI (Next Experience), and it is active on both the instances. Any other possibility ?
