issue in rendering HTML in Outlook

wanted to display this template on the outlook as the user clicks on the survey notification but i am unable to create this format on outlook

If you are trying to acheive this using mailto protocol, then it won't work.
Unfortunately, the mailto protocol does not support HTML content in the body of the email. The body content must be plain text.
As an alternative, you can create a survey with smiley faces and add the link as hypertext in your email. Ask the user to submit the survey. Based on the survey response, you can trigger a notification to the fulfillment team.

so email is sent to user and when they click on the link in email then it should open Outlook with the above UI?
If you are using mailTo attribute for this -> If yes then it's not supported by mailTo attribute in HTML
It's not ServiceNow limitation but limitation as per RFC standard RFC 2368
The notification is not generating emails with html content when using mailto links
