Knowledge Base - Actionable Feedback Help - Please respond to this post

Good day!
When you enable these choices for a Knowledge Base - When I tested to see if Rate this article, Helpful?, and Flag this article, the actionable feedback box only appeared for Rate this article and Helpful?. Does anyone know if the Flag this article option does not allow this same popup box feature to provide feedback like the other two options?
1. Checked yes to turn on this property -Property Name - Create actionable feedback task when an article is flagged. [Note: This property is applicable to portal and workspace only.]
2. Checked yes to turn on this property - Property Name - Create actionable feedback task when an article is marked as not helpful. [Note: This property is applicable to portal and workspace only.]
3. Input a number in this property - Property Name - Create actionable feedback task when an article is rated at or lower than this value. 0 or no value indicates that actionable feedback tasks are not enabled for Rating type feedback. [Note: This property is applicable to portal and workspace only.]

Is this happening for all articles and knowledge bases?
Is this happening for all users?
Can you please Check if ui macro "kb_view_common_header_toolbar" is customised?
The properties you mentioned above are for creating feedback tasks. and they are not related to the pop up box.
