Hi Team,
My requirement is to display a login page with ok and cancel button, whenever approve option is selected in 'required approval' custom field on incident form. After filling the login details system check the user details from user table.
How to proceed?
Any help will be appreciated.

I'm not 100% sure, but you could use a modal for this. In the modal, you can add the username and send that information to the backend. However, I'm still unclear about what would happen if the information doesn't match. We may need to consider how to handle mismatched data.

the user is already authenticated within the instance as that user is able to change the Group
Why extra check required? this is unnecessary

but why?
the login will be taken care by instance itself or your SSO

I just read your requirement you need credentials again for username and password. But this is OOB feature in "ESignatureUtils" plugin - or similar on your system, you can just enable for incident. We are using it for change. It's linked to SAML SSO.
And just to tell you, username and password is "Side Door Login" Username and Password - that is not the same password that people enter in SSO login. So, even if you are going by (modal way, or widget way or a UI Page way) - several ways to implement it - this will be stuck somewhere.
