Major Incident Management - SMS Notifications?

Hello - I have Notify successfully setup. To be sure, I manually sent myself an SMS and it's working.
I am trying to get Notify set up with Major Incident Mgt - to be specific, the Incident communication plans.
First, I set up my user contacts so that I am the Incident Manager - meaning I show up on the User Contacts tab of the Incident Communication Plan when I create an Incident Communication Plan from an incident.
In the Notes section of the incident communication plan, it even says an SMS was sent to me ,but I never got it. I am getting SMS for other things - such as On-Call and when I manually send myself an sms, so I have NO idea why I wouldn't be getting this SMS.
Any ideas???

I actually noticed that the Send SMS/Initiate Conference Call related links are not displayed on the Incident Alert form... i think this might be the problem. But I am not sure how to get them to show.
