Make a questionnaire, link it to a button in SOW and store the results

I want to create a button that is available in the SOW, as well as the classic UI and when click on it, a questionnaire will be opened. In this questionnaire, I will have the questions, with the answers yes or no. If the choice is no, another field with appear, it will be mandatory and it will require to write in it. How can I achieve this and store the results in a table? Maybe assessment table.
