Missing Comments in Service operations workspace.

Hello Troubleshooters,
I'm missing "Comments"  in Compose pane in SOW(service operations workspace) and I don't see "toggle compose setting". Currently I'm in Utah p4.
Please find the attachments for PDI and affected Instance would explain the Issue. Any Inputs would be much appreciated.

Did you check is there any UI Policy or client script hiding comments field?

Yes I would make sure the comments field is present on the form view you're presenting in workspace and not being hidden by anything on page load.
