Is there any way to modify an existing running Legacy Workflow or Flow Designer which then updates existing workflows/flows ?
Suppose I have a record that has an attached Legacy Workflow. It has 3 workflow activities that creates a child task. If I were to make an update to that workflow and publish a new version (for example, add a 4th activity), it of course does not retroactively update all versions of that workflow.
In the scenario above, you could cancel the existing workflow and re-attach with the new version but it would create new child tasks (for possibly tasks that already have been completed). Im not sure if there's an easy or best way to handle this.

Technically, yes, but the new changes will only apply to new records unless you cancel the old attached workflow and trigger it again. Otherwise, there is no other way.

yes if you attach the new version by cancelling the older, it would create new child tasks.
If you do so you will have to identify the newly created tasks and discuss with customer if those are to be closed since those were already completed earlier.
