Multi Level Pivot, Percentage, Counts in a report

Is it possible to achieve this sample report in ServiceNow?

This is how close I can get so far using PA Scorecard.

So you are not going to be able to get that exact format but you will be able to get close/part of it.  You need to create the indicators for the counts and percentages and then create a widget that is of type List and probably Visualization of Scorecard. You can then have it in rows.

You can look at the "Basic indicators" widget that is on the Incident Management dashboard for an example.

If it absolutely has to be in that format then you will have to make a UI macro I think.
