Multiple output with the same condition in decision matrix

HI 

 

i want to create a new case when we have same condition but the different output for the condition in decision table 

 

eg

There are 2 record with same condition but different result 

i want to create a new case whenever for different result in flow designer

@Ankur Bawiskar can you please help it out
