My Assessments & Surveys Issue

Hello,

I am trying to test the Assessment I generated however, it is not giving me the usual faces for (Very Dissatisfied, Dissatisfied, Neutral, Satisfied, Very Satisfied) where I can put my rating on.

The right side of the assessment form is just blank.

@tindiz In your case you can skip the Template creation and use the existing one. In survey Designer you can use the existing Template like "Smiley Faces" you will get the same output as below,
