Need a new popup form in change same like a Risk assessment

Hello All,

I have a requirement to implement New pop-up form on the Change Form called "BCA Freeze Exception Questionnaire" under Related Links section , which is  similar to the GSEP Change Risk Assessment pop-up form ,and this new pop up BCA exception questionnaire have a different questions and the rest of the things are same as risk assessment.

Could someone help me on this .

You need to create a new UI action and select only related Link as check box to appear on form.
